Stop Memorizing Birth Positions: What Your Body Actually Needs for Labor

If you've spent any time on pregnancy Instagram, you've probably seen some version of this:

Baby is high? Do THIS.

Baby needs to engage? Open the inlet.

Stalled at 7 centimeters? Try THESE three positions.

Ready to push? Open the outlet.

And listen, I love birth biomechanics.

I've invested a lot of time learning about the pelvis, movement, alignment, compensation patterns, fetal positioning, and how all of those things interact during birth.

But sometimes, in our effort to make biomechanics easy to understand, we accidentally turn it into another set of rules pregnant people think they're supposed to memorize.

The goal isn't to walk into labor with 37 Instagram positions stored in your brain! The goal is (or should be) to understand and prepare your body well enough that when labor begins, you can work with it.

Birth Isn't a Choreographed Routine

There absolutely are biomechanical principles that can help us understand labor.

Your pelvis isn't a rigid tube. It's a dynamic structure with joints that allow subtle changes in shape and dimension.

The positions of your legs, pelvis, spine, and sacrum can influence the space available at different levels of the pelvis. Research (including biomechanical modeling) supports the idea that maternal positioning can change pelvic dimensions, while broader evidence supports mobility and freedom to choose positions during labor.

But “External rotation can help create more space at the inlet” is not the same thing as “At this point in labor, everyone should externally rotate their hips.”

Bodies aren't standardized!

Imagine someone who already spends much of her day standing and walking with her legs externally rotated.

Her body has gotten very good at accessing that pattern. What might be more useful during pregnancy and labor isn't endlessly reinforcing the movement she already has…it may be exploring the movements she doesn't access as often.

Can her hips internally rotate? Can her pelvis move independently of her rib cage? Can she shift her weight comfortably in both directions? Can she move between these patterns without having to think hard about them?

This is where biomechanics becomes much more interesting than just “Do this position to open your pelvis.”

Your Body Arrives at Birth With a History

None of us arrive at pregnancy as a blank anatomical diagram.

You've spent decades developing movement patterns. Maybe you always stand on one leg. Maybe your ribs tend to flare. Maybe you have tremendous flexibility but struggle with stability. Maybe an old injury changed the way you load one side of your body years ago.

These aren't necessarily problems. They’re information, a map, a puzzle that nerds like me love to put together to see - what’s really going on here?

Our bodies are extraordinarily good at adapting. We compensate constantly so we can move through our lives.

And those adaptations are one reason a generic list of “best labor positions” can only take us so far.

A childbirth educator teaching a room full of pregnant people cannot possibly assess every person's mobility, alignment, history, strength, compensation patterns, comfort, baby's position, and what labor will ultimately ask of them. Trust me, I’ve tried.

That’s why I think individualized birth preparation and comprehensive childbirth education complement each other so beautifully - each provides something the other cannot.

The Other Problem With Memorizing Positions: Labor Brain Is Different

There's another reason I don't want you spending labor mentally scrolling through the reels you've saved: labor is not an intellectual exercise.

Many people describe moving deeper into labor as becoming increasingly inward, less conversational, less aware of time, and intensely focused on what is happening in their body.

Researchers have begun studying this phenomenon as “birthing consciousness”, an altered state during physiological birth with similarities to the psychological state of flow.

A 2024 study of 766 women found that all nine dimensions commonly associated with flow could be identified in childbirth experiences, including intense concentration, merging of action and awareness, altered perception of time, and reduced self-consciousness.

This is fascinating emerging research! It describes something birth workers have observed for a very long time: ideally, at some point, people in labor stop thinking their way through birth and begin being in it.

And I don't particularly want to pull you out of that state every twenty minutes to say: "Okay! You're probably at the midpelvis now. Remember which position opens the pelvis here?"

I want your thinking brain to have done much of that learning before labor, so that any suggestions about positioning are easy to access without involving your thinking brain too much.

Train the Possibilities, Not the Choreography

This is one of the things I love about the Body Ready Method® approach.

The purpose of practicing movement during pregnancy isn't simply to memorize positions, it's to create options.

Body Ready Method describes preparing the different levels of the pelvis and practicing movements prenatally so that those patterns are familiar enough to be accessed more intuitively during birth.

We're not rehearsing: At 4 centimeters, do A. At 7 centimeters, do B. At 10 centimeters, do C.

Instead, we're building a rich, varied movement vocabulary that helps you prime your intuition and body awareness.

When you spend months exploring different birth movements, getting to understand your body’s held patterns, and getting curious about your unique mobility and alignment, a list of possible birth positions suddenly becomes an opportunity rather than a checklist.

Now those movements aren't foreign concepts someone is introducing to you for the first time while you're contracting.

Your body has been there before, and you have valuable information about what might work best for you that gives us somewhere to start.

Where Body Awareness Becomes Birth Preparation

The other half of this work is learning to notice.

What feels restricted?

What feels spacious?

What makes you brace or tense up?

Does one side feel completely different from the other?

What happens if you stop trying to make a position look “right” and instead get curious about what you actually feel?

That skill has a name: proprioception, or the ability to perceive and interpret the movements of our muscles, limbs, and joints.

Research specifically connecting proprioception to labor is still developing, so I wouldn't tell you that improving it will make your labor faster or easier.

But we do have interesting evidence around mindfulness-based childbirth preparation.

A small randomized trial found that mindfulness-based childbirth education improved childbirth self-efficacy and mindful body awareness compared with conventional childbirth education. Other randomized research has found improvements in fear of childbirth, pain acceptance, and mindful awareness.

Anecdotally, I can tell you that the parents I work with who really take the time to get curious about their bodies have a MUCH easier time letting go and trusting themselves in labor. No micromanaging needed!

I don't just want you to know what movements exist. I want you to develop enough awareness of your body that you can begin asking: What do I need right now?

So...Do Birth Positions Matter?

Absolutely! Understanding birth biomechanics can be incredibly empowering.

If labor stalls, if baby seems to be having trouble descending or rotating, if something doesn't feel right, or if you simply want another option, having someone in the room who understands biomechanics can give you useful tools to experiment with.

But a position is a tool, not a prescription.

Biomechanics gives us hypotheses. Your body gives us information, labor gives us feedback, and then we adjust.

Two Approaches to Birth Preparation

This is why my birth preparation work tends to happen from two directions.

First, we get specific.

In one-on-one Body Ready Method® work, we can look at your body.

  • How you stand.

  • How you move.

  • Where you have options.

  • Where you compensate.

  • What feels easy.

  • What feels unavailable.

And then we can spend pregnancy expanding your movement vocabulary so that you're entering labor with more possibilities, rather than waiting until something feels stuck and reacting to it.

Second, we practice listening.

(This is something I weave into prenatal yoga and childbirth education, too.)

I want you to understand your body and the principles behind different positions, so you know why we might choose one movement over another.

But ultimately, I want you to be able to stop thinking about it.

To move, breathe, notice, and respond, to trust the sensations you're receiving from your own body.

And if you need help, then we bring our knowledge of biomechanics into the room and get curious together.

Not “You're at this stage, so you're supposed to do this.”

But “Hmm. What is your body telling us? What happens if we try this?”

That is the biomechanics I love.

Responsive. Individualized. Curious.

Because there is no single perfect position for labor.

There is only another tool we can use to help you understand (and work with) the body that's actually giving birth.
